[Mpi-21] Review of MPI-2.1 combined document

The MPI standard has several references to ANSI C and on page 15 of the
combined doc we say to read ANSI C as ISO C. Does it make sense to fix this
and just refer to ISO C?  We might want to mention that the original books
said ANSI C and the combined book has rectified that
