[Mpi-21] Review of MPI-2.1 combined document

What about the usage of deprecated functions?
(for example, the old attribute functions, old datatype constructors, ...)

Since the code examples were not modified, they often still use versions
that were deprecated in later versions of the standard.
(see pg. 17 in the merged document)

Examples (by no means complete list):

MPI_Attr_get: 309, 229, 244

...
