[Mpi-forum] Final document of the night

I've updated the credits list - please check.

There are a number of problems with the indices. In the example indices, removed functions such as MPI_Type_struct show up. In the MPI Function index, there are names in mixed case; this index is only for the language-independent functions. This usually means the C binding was used where the language-independent (all uppercase) was intended; sometimes it means that the wrong macro was used for a language-specific instance. These need to be corrected.

There are still invalid references to C++, especially page 380, line 30.

p 790, line 2 "where" should be "were" (in the change log, entry 5)

This resulted in finding a LOT of little issues (and a few big ones). If chapter committee members were in the room, they fixed and committed the text. But for committees that had no representatives in the room, we made the executive decision to make the fixes ourselves, review them, and then commit them.
