[Mpi-forum] Final document of the night

I've updated the credits list - please check.

There are a number of problems with the indices.  In the example indices, removed functions such as MPI_Type_struct show up.  In the MPI Function index, there are names in mixed case; this index is only for the language-independent functions.  This usually means the C binding was used where the language-independent (all uppercase) was intended; sometimes it means that the wrong macro was used for a language-specific instance.  These need to be corrected.

There are still invalid references to C++, especially page 380, line 30.

p 790, line 2 "where" should be "were" (in the change log, entry 5)
