#278, #303, and #304 are not related to C++ at all, but about deprecated MPI-1 routines.

#278 updates the examples throughout the document to use the current functions, rather than the deprecated ones.
#303 moves some of the deprecated MPI-1 routines to a new deleted chapter (all but attribute related).
#304 moves the attribute related deprecated MPI-1 functions to the new deleted chapter, but is not ready yet.

#278 and #303 are ready for a formal reading, but are not on the agenda, pending available time.  I moved them to the 'scheduled' state because if we do have time, we will read them - they are ready.
