[Mpi-forum] Final version of cleaned up Chapter5 (CollectiveOperations) including NBC

BTW, what is the status of the nonblocking version of
Reduce_scatter_block, which was introduced in 2.2? It wasn't in the
original nonblocking collectives chapter we voted on, but it needs to be
added at some point for MPI 3.
