[Mpi3-hybridpm] Sample implementation of MPI Helper Team proposal

I've just updated MPI Forum ticket #217 with instructions for accessing a
sample implementation of the proposal for MPI_HELPER_TEAM.

The code has been pushed to the ANL repository for DCMF. The implementation
runs on BlueGene/P systems (DCMF, MPICH2).
