[Mpi3-hybridpm] Updated chapter draft

All,

I've uploaded a new draft of the hybrid chapter to the wiki:

It contains most of the edits we discussed in the previous telecon and 
the following emails (some of the parts are left as comments, as they 
need more discussion). We still need some better examples for the shared 
memory stuff.

Please take a look to make sure I didn't miss anything.
