[Mpi-forum] MPI user survey

Jeff, looking at the survey I noticed that when I go to a survey page 
the top has links to "Back" and "Exit Survey".  If you follow the "Back" 
link that page ends up not having a "Back" link.

I was also wondering for the questions that have "Select all that apply" 
whether there is a way to have the "I don't know" selection reset the 
other selections.  Plus should the "I don't know" selection occur after 
"Other"?
